you will need to complete prior to your first visit. By filling out the
forms in advance, we will be able to attend to your dental needs more
quickly upon your arrival. But, if it is not convenient for you, we have
set time aside for you to complete the forms in our office at your
appointed time. Please remember to bring a list of medications with you
to your appointment and any other pertinent information that will ensure
